The current model of medication management and reconciliation typically sees physicians and nurse practitioners taking the lead on these critical processes. While prescribing medications should be part of a physician's responsibilities, physicians and other clinicians are often not in the best position to optimize a patient's medication plan. Rather, these responsibilities should be assigned to clinical pharmacists.
During this panel discussion, moderated by Greg Downing, DO, PhD, Cureatr’s Chief Clinical Officer, our clinical pharmacy team discusses why clinical pharmacists possess the training and expertise required to perform the highest quality medication reconciliation and other medication-related assessments. They also cover the advantages of clinical pharmacists providing medication services; give insight into how providers can better integrate and leverage clinical pharmacists as a part of the care team, particularly during transitions of care; and share their unique experiences working with patients and care teams to improve medication-related outcomes.
