Cureatr and Unite Us have collaborated, connecting health and social care. The uniting of these two companies strives to solve Social Determinants of Health and the complex medication management equation.
Together, Cureatr and Unite Us can more holistically care for patients and evolve the role of Cureatr’s certified community health workers (CHWs). This partnership will provide services to address medication access issues, and the new health equity program allows CHWs to screen patients for social determinants of health (SDoH) insecurities related to food, transportation, housing, and health literacy and connect them to resources in their community.
