Cureatr and Unite Us partner to connect health and social care. The uniting of these two companies strives to solve Social Determinants of Health and the complex medication management equation.
Together, Cureatr and Unite Us has more holistically cared for patients and evolved the role of Cureatr’s certified community health workers (CHWs). This partnership has provided services to address medication access issues, and the new health equity program allows CHWs to screen patients for social determinants of health (SDoH) insecurities. Currently, we have helped solve issues related to the following:
- Transportation to the pharmacy or health appointments
- Purchasing meds or skipping health appointments to save money
- Health literacy barriers
- Patients determining where their next meal came from
- Those who did not have stable housing
